2. Medical Evaluations

  • Be performed by a Physician or Other Licensed Health Care Professional (PLHCP)
  • Use the mandatory medical evaluation questionnaire (Appendix C)
  • Be confidential between the employee and the PLHCP

  • The PLHCP, supervisor, or program administrator recommends re-evaluation
  • Information from the program indicates a need for re-evaluation

3. Fit Testing Requirements

Fit testing ensures that the specific make, model, and size of respirator creates an adequate seal with the wearer's face.

Initial Fit Testing

  • Required before first use

Annual Fit Testing

  • Repeated at least annually
